We are pleased to announce that we’ve been selected as a Patient-Centered Medical Home designated practice by Blue Cross Blue Shield of Michigan. Designation is a rigorous process undertaken voluntarily.
You may have noticed some changes in our practice over the last two years. These changes have been made to improve the way we provide care to you and are in keeping with the concept of the Patient-Centered medical Home (PCMH). A PCMH uses a team-based approach to care management; uses enhanced technology to assure consistent, high-quality care; and provides expanded access to services. We will maintain our focus on you, our patient, as we continue to improve the way we work with you to meet your health care goals. We want you to feel “at home” in our practice.
